Chiltern Way Academy Wokingham supports students aged 8-16 with ASC, providing a caring and forward‑thinking environment designed to nurture confidence, resilience and academic progress. We are seeking a committed and adaptable Transition Teacher to support pupils at key points in their educational journey, helping them successfully access learning and thrive within a specialist setting.
For the right candidate, there may be the opportunity to take on additional SENDCo responsibilities.
About the RoleYou will play a vital role in supporting pupils’ transition into and across our provision, delivering high-quality teaching tailored to individual needs.
Key responsibilities include:
- Planning and delivering engaging, differentiated lessons, including individualised programmes aligned to the National Curriculum
- Supporting pupils’ academic, social and personal development during key transition stages
- Creating a safe, structured and inclusive learning environment that promotes positive behaviour and wellbeing
- Contributing to curriculum planning, assessment, recording and reporting
- Working collaboratively with colleagues, families and external agencies to ensure a joined-up approach to support
- Playing an active role in pastoral care, acting as an advocate for pupils and promoting their progress and participation in school life
You will also contribute fully to the life of the school, maintaining high standards of teaching, learning, and classroom environment, and engaging in ongoing professional development and school improvement activities.
About You- A qualified teacher with experience or strong interest in SEND, particularly ASC and SEMH
- Skilled in delivering adaptive and inclusive teaching strategies
- Committed to supporting the whole child—academically, socially and emotionally
- An excellent communicator with strong organisational and teamwork skills
